These four cases of prolactinoma illustrate the different ways in which this condition can present. In each case, investigation and treatment have been successful, and yet the best therapeutic approach remains controversial. Prolactin excess is a common condition, and the techniques available for its investigation and treatment have undergone major advances. It is the fundamental biology of prolactin excess which remains poorly understood. More sophisticated investigative techniques, such as IÇMR imaging, and increasing information about the natural history of prolactinomas, with careful long term follow -up of patients such as these, will hopefully provide answers to the questions raised in this discussion.
